Parents will have three options for registering their child/children in CMHA this year:
1. On-line registration for all programs will begin starting Monday, June 7, 2010. If you chose to register on-line, you may choose to pay with Mastercard or Visa or by mailing a cheque or Money Order to the Registrar. Please note that in addition to providing all your information on-line you will have to send a photocopy of your child’s birth certificate to the following address: Registrar, Canmore Minor Hockey Association, P.O. Box 8266, Canmore, Alberta, T1W 2V1, to complete your registration. This copy will be kept on record for future years with CMHA. The deadline for on-line registration is: Wednesday, June 30, 2010. After this date, registrations will still be accepted but space may be limited. To register on-line, check out the information under the On-Line Registration Tab.
2. For those of you who prefer not to or are unable to register on-line, we still have a registration night at the Canmore Recreation Centre Foyer on Wednesday, June 16, 2010, from 7:00pm – 9:00pm. You must bring a photocopy of your child’s birth certificate to the registration night (this copy will be kept on record for future years with CMHA), your child’s Health Care Number, and a cheque, cash, or money order to complete your registration.
3. If neither of these options work for you and you need to send in your child’s registration by mail, please ensure that it includes the following: (1) a completed Registration Form, (2) a completed Internet Disclaimer and Consent Form, (3) a completed Volunteer Form, (4) a cheque or money order, and (5) a photocopy of your Child’s Birth Certificate (this copy will be kept on record for future years with CMHA). All mailed applications can be sent to: Registrar, Canmore Minor Hockey Association, P.O. Box 8266, Canmore, Alberta, T1W 2V1. The deadline for receiving applications is Wednesday, June 30, 2010. After this date, registrations will still be accepted but space may be limited.
Payment in full for all programs is due by Friday, September 3, 2010. Any payments received after this time will be subject to a $50 late fee and will be treated as a late registration. In this case, the Board has the right to place the player based on program availability. If payment for any program is not received prior to the commencement of the Hockey Evaluations for kids trying out for a CAHL League Team or first practice for the kids enrolled in the House League, your child will not be permitted to play until payment is received in full.
